2026 Interfaith Health & Wellness Summit Grant Application
Purpose of grant:
Anthem recognizes that houses of worship and faith-based organizations are trusted entities, providing spiritual refuge and a place of hope and help during good and challenging times. We appreciate and value your work and are here to support your health and wellness initiatives that are helping improve the communities you serve.
The 2026 Interfaith Health & Wellness Grant aims to provide financial assistance to a house of worship or a faith-based organization that implements a health and wellness program for their faith assembly or local community. The program must be implemented between 01/01/27 – 12/01/27.
Grant recipients may become eligible for an It is Well Community Star Award if their program meets one of the following requirements and demonstrates measurable results:
Addresses state health priority:
Immunizations and vaccine access
Substance use disorder prevention, treatment, and recovery support
Infant mortality and maternal/child health
Smoking cessation and tobacco/vaping prevention
Obesity prevention and health weight initiatives
Mental and behavioral health
Chronic disease prevention and management
Nutrition education and food access
Physical activity and active living
Health education and health literacy
Social determinants of health, including housing stability, transportation, employment, and access to care
Family wellness and caregiver support
Senior health, aging supports, and chronic condition management for older adults
Target populations:
Rural communities
Unemployed or underemployed individuals
Diverse and historically underserved communities
LGBTQ+ individuals
Individuals with disabilities
Individuals re-entering society from incarceration
Children, mothers, and families in the foster system
Aging Hoosiers
Pregnant and postpartum individuals
Grant request amount: Up to $1,000
