Pilgrim’s Progress CDC, Fort Wayne

Grant will fund the program: “Faith & Flow Yoga: Reducing Obesity in East Central”. Faith & Flow Yoga is a year-long, church-based wellness initiative designed to reduce obesity and improve health outcomes for residents of Fort Wayne’s East Central neighborhood.

 
 

Castleton UMC - Still Waters Adult Day Center, Indianapolis

Grant will fund the program: “Stories that Connect: Intergenerational Storytelling for Seniors”, in partnership with Arts with a Purpose. The funding will cover program delivery costs by compensating experienced storytellers in engaging older adults and fostering intergenerational connections.

 
 

Nazarene Missionary Baptist Expungement Ministry, Evansville

Nazarene Missionary Baptist Church Expungement Ministry serves individuals in Vanderburgh County through its Expungement Program. This grant will support this ministry to host a re-entry resource fair in April 2026.

 
 

Mercy Table, Inc., Anderson, IN

Grant will support the Stirring Up Success Program. This is Mercy Table’s innovative, hands-on program designed to reduce obesity risk and improve health outcomes in diverse families facing food insecurity, chronic life stressors, and limited access to fresh, nutritious foods.

 
 

St. Matthew Frankfort UMC, Frankfort

This grant will support the “Know Your Numbers and Smoking Cessation Programing”. St. Matthew seeks to provide a twice-monthly health screening and wellness education program that includes smoking cessation support and “Know Your Numbers” health checks. This request supports the resources needed to provide monthly health screenings for individuals in substance-use recovery and seniors in our neighborhood.
